What to Expect &
Bring to Your First Visit

We ask that new patients arrive 10-15 minutes prior to the time of the appointment to check in and sign any necessary paperwork. Please bring your completed New Patient Form and Release of Records Form as well as your health insurance card (if applicable).

A staff member may review the information you’ve provided with you prior to seeing Dr. Tahir.

Additional Payment Information

We accept most insurance providers, including most PPO plans. Payment options for expenses not covered by insurance will vary from treatment to treatment, but we are always transparent about cost before we move forward with any procedure.

Initial Consultation – $150.00
Established patient visits – $115.00

Self-pay for in-office procedures ranges from $300-$800.

*Excisions of lesions such as cysts and lipomas are billed based on location and size.

Upon arriving you are greeted by Faye and Michelle who are absolutely the best front office personnel. Helped to make sure the insurance plan was approved for the surgery and explained any potential out-of-pocket expenses. Not to mention they have a great sense of humor. 🙂

The medical office offered a friendly and clean atmosphere as you wait to see Dr. Tahir. There was fresh fruit and complimentary coffee.
